Automatic Debit Terms and Conditions

General Information

The processor Gilden Family Chiropractic uses for the Automatic Monthly Plans is http://www.gocardless.com.

All plans are month to month with no long term commitment.

All plans require a 30 day notice to cancel.

Fees

There are no additional fees to use any of the automatic debit plans.

The patient is responsible for selecting the plan that meets their needs.

Hours and Locations

The office hours and locations are posted on the website.

Availability to see the Doctor

Dr. Ken Gilden will be available any day and time the office is open.

The doctor is also a volunteer firefighter and may be out of the office for emergency calls and training.

Patients are encouraged to sign up for text notifications to inform them of planned and unplanned days the office will be closed.

Signing Up for a Plan

Patients can sign up by going to the Payment Plans page and select the plan that fits their needs.

A confirmation email will be sent to the patient with information regarding their plan.

Canceling a Plan

Plans can be canceled at any time with a 30 day notice.

Patients must email Dr. Ken Gilden at drgilden@gmail.com and include their name and phone number in the body of the email.

The doctor will send a confirmation email of the cancelation and so will the processing company.

If the patient does not receive an email confirmation with in 3 business days they will need to resend the cancelation notice.

If the patient has any issue with cancelation of a plan Dr. Ken Gilden will handle the problem personally.
